This is a friendly reminder about the Public Servant Award Program.

 

Attached above are the Public Servant Instruction Sheet and Entry Form that VFW Posts may use to help obtain nominations from their local Police Headquarters and /or Fire Departments. The entry form should be forwarded on with the required documentation of the Posts winner submissions, one in each category, if applicable, to you by January 1 to be entered into your Department judging.

 

Below is the Department Headquarters link for the "VFW Department Public Servant Award Request Form” for your Public Servant winner in each category, if applicable. As this is a Formstack form, once you have completed each form, click SUBMIT and the form will be forwarded to National Headquarters to begin the process of preparing your winners citations. Once the citations are completed, they will be forwarded to you at Department Headquarters for presentation. Please be sure to notate the presentation date on this form, if unknown at the time of submission, please list TBD and once the date is known, please contact Tammy Beauchamp via e-mail listed below.
