VA Maine Healthcare System Alert: Abbott Glucose Monitor Sensors

VA Maine has been made aware of a potentially high-risk issue involving Abbott glucose monitor sensors. The FDA has issued an Early Alert for the following products:

• FreeStyle Libre 3
• FreeStyle Libre 3 Plus

If you currently use one of these Abbott glucoses monitor sensors, please take immediate action to determine if your device is affected.

What You Need to Do

1. Check Your Sensor:

• Visit www.FreeStyleCheck.com and select "CONFIRM SENSOR SERIAL NUMBER.”
• You will need your sensor’s serial number, which can be found:
• In the app or reader (if currently wearing the sensor)
• On the label at the bottom of the sensor applicator or carton
• In the user manual (if using a connected insulin delivery device)

2. If Your Sensor Is Affected:

• Immediately discontinue use and safely dispose of the affected sensor.
• Request a free replacement through www.FreeStyleCheck.com by entering your serial number and contact information.

3. Monitor Your Blood Glucose:

• Use a blood glucose meter or the built-in meter in your FreeStyle Libre 3 Reader to make treatment decisions if your sensor readings do not match your symptoms or expectations.
